ATTENTION: Medical students who are tardy to class will be absent from a class or exam must complete this absence form.

                    

• Absence Request from Mandatory Class - No Advance Notice – Use to request an absence in a mandatory class WITHOUT ADVANCE NOTICE, such as an unexpected illness or emergency.  Mandatory classes are those where attendance is required (e.g. small groups, labs, problem solving and colloquium).  

 

• Absence Request from Mandatory Class - Advance Notice – Use to request an excused absence in advance for a Mandatory class. Please request the absence as soon as you are aware of it, and as far in advance as possible.  

 

• Request Excused Absence from Exam for Non-Medical Reasons – Use to request an excused absence from an exam when there are non-medical reasons.  

 

• Request Excused Absence from Exam for Medical Reasons – Use to request an excused absence from an exam when there are medical reasons. The student must also submit a note from their provider. BOTH forms should be included in your e-mail.  If your provider does not have a standard note/form, please use the following  Doctor's Note to be completed and signed by your physician.
